Dentro de los motivos de consulta, el dolor abdominal es
uno de los más frecuentes y desafiantes. Esta población
tiene características propias que dificultan el diagnóstico
y tratamiento. El enfrentamiento del médico ante el dolor
abdominal debe ser distinto al de los pacientes más jóvenes.
El clínico debe tener siempre en mente que estos pacientes
se presentan tardíamente, con signos y síntomas atípicos.
Dado el amplio espectro de urgencias abdominales en los
adultos mayores es de utilidad ordenar los diagnósticos
diferenciales de acuerdo al mecanismo fisiopatológico
subyacente en: inflamatorias, obstructivas, vasculares y
otras.
Ante un paciente adulto mayor con dolor abdominal no se
debe dudar en complementar el diagnóstico con exámenes de
laboratorio e imágenes y se debe optar por una política de obser-
vación previo a la decisión del alta.
